In Personal Health:

1. Because many diseases are preventable or curable, it is important to evaluate your values, beliefs, behaviors and performance abilities that affect health and wellness. Make a list of all positive behaviors that you practice to enhance your health status (such as exercising and eating healthy). Now, make a list of all negative behaviors that diminish your health status (such as smoking and not getting enough sleep). Beside each positive and negative behavior, list predisposing, enabling and reinforcing factors that influence that behavior and behavior-change decisions.

2. Based on your answers from Question #1, select a negative health behavior that you would like change. What risks are involved if you continue your present behavior now and/or in the future? What can you do right now to enhance your health and wellness? List the steps you intend to take today, next week, throughout the month. Fill out a behavior change contract (Located at the beginning of the text) to help you set your goals, anticipate obstacles, and create strategies to overcome those obstacles.
