Apply gown, goggles, mask if risk of splashing. Remove dressing one layer at a time, observe for drainage. If moist to dry dressing sticks, gently free dressing. If dry dressing sticks, moisten with normal saline and remove. Assess wound for appearance, color, size, depth, drainage, edema, drains, approximation, granulation, odor. Fold dressing with drainage turned inward and remove gloves inside out. See procedural guideline 24. 1, p. 586, pp&e, or wound assessment sheet in this manual. Provides containment of soiled dressing and reduces transmission of microorganisms. Pour cleansing solution in basin or open swabs. Clean from least contaminated to most contaminated area. If drain present, cleanse using a circular motion starting at the drain site and moving outward. Use antiseptic swabs or gauze dipped in solution. Use a new swab or gauze for each cleansing stroke.
