Placebos suggest that there are other things that contribute to how you feel. There might not be better placebo responders but better contexts where people will be more receptive. There is no evidence that placebos can affect the disease process, but they will affect the symptoms. The effect is the way of describing the interaction between the patient and the provider. Not ethical in clinical care if the client is not aware. The effect is about everything about the environment of the provider. One of the main contributors is the empathy and emotional support that goes on between a therapist and the patient: this is a critical part of the pill you receive and its effectiveness. The effect relies on the trust between one and its provider.
