Rationale of incident investigation: incident investigation is an important component in rac model, which in turn is a integral part of a health and safety program, benefits of incident investigation i) Investigation uncover the direct causes of the incident. Thereby allowing of the subsequent exploration of corrective measures ii) Example: direct cause of an incident may be inadequate safeguards on equipment. However there may also be contributing factors to result an incident. Loosing clothing on employees and lack of instruction in the proper procedure for equipment use can be the contributing factor. iii) Corrective measures, such as training or equipment design improvement can be implemented to prevent similar incidents happen, once direct and contributing causes are identified iv) The report generated by an investigation can be used by hr and safety specialists to identify trends.
