Incident: flow of incident that doesn"t result in a loss or a near miss. Important component in a hazard recognition, assessment, and control (rac) program. Integral part of a health and safety program. Benefits of incident investigations: (study by p. laing 1992: determine direct causes. Identifies contributing causes: prevents similar incidents, creates a permanent record, determine cost, promotes safety awareness among employees. Incident investigations are influenced by: timing: critical factor in incident investigation; affects several types of information, severity: companies tend to examine only those incidents that have the most serious consequences, legal requirements- 1st aid, med. When investigating an incident, the hr or safety specialist should concentrate on: human factors, situational factors, environmental factors. Studying the workers as a source of incidents does not mean that the investigator is looking for a scapegoat. Intent is to collect facts, not assign blame. Analysis of unsafe conditions that led to the incident is a critical step.
