The therapist should remember that not all distressing thoughts are inappropriate: there are times when thoughts are realistic, and the therapists job is not to introduce unrealistic positive thoughts. Instead, it is recommended you use problem-solving skills to minimize the likelihood of the undesirable outcome, or help the person look at the meaning of the outcome and how they can cope with it. Clients need a rationale, because you will often be asking them to focus on the most frightening, most depressing, or most shameful aspects of their lives. It is important the client understands the term cognition : a cognition is a thought or visual image that you may not be very aware of unless you focus your attention on it.
