A nurse-led, evidence based, home visitation program that improves the health, well-being and self sufficiency of low-income, first time parents and their children . Improve pregnancy outcomes: work with moms and encourage better prenatal care. Check ups, diet, nutrition, vitamins, reduce drug/alcohol/tobacco use. Improve child"s health and development: educate parents about how to provide responsible care. Create an environment that"s positive/nurturing, teach parents first aid. Improve economic self-sufficiency of family: develop goals and a future for family, make healthy choices about finding work/education, choices about significant others, and planning subsequent pregnancies. Highly trained nurses serve as home visitors: caseload- less than 25 families. Therapeutic relationship between mother and nurse is foundation for intervention: client-centered- personalized information for mom, strength-based- her strengths and what she needs to work on. Personal (maternal) health: nutrition, exercise, substances, mental health. Environmental: how safe is the home and neighborhood environment. Life course development: family planning, birth control, job search.
