Determine root cause - the emphasis of incident investigation should be to find the root cause of the incident so you can prevent the event from happening again. Identify contributing causes - always look for deeper causes. Do not simply record the steps of the event. Benefits of incident investigation: determines direct causes, prevents similar incidents, creates a permanent record, determines cost, promotes safety awareness among employees. These steps can be used to investigate any situation (e. g. , where no incident has occurred yet) as a way to prevent an incident. Given that investigations are time-consuming, companies tend to examine only those incidents that have the most serious consequences. Events (e. g. , injury requires medical aid or results in lost time) must be reported to a workers" compensation board, normally within three days.
