Lecture 8: healthcare integration- the role of primary care. What are the issues: health systems lack coherence and coordination, primary care is touted as a possible solution to this problem. Many rationales exist for the conclusion: primary care as entry point, primary care as the coordinator/gatekeeper, primary care as the long term relationship, primary care as having the more holistic view of health and the individual. System is gaining complexity due to sub-specialization results in inward-directedness at expense of integration. Key issues are at point of interface: Canada"s performance compared to others: canada ranks below many developed countries in: timely access to care, after hours care, ed use, use of non-physician professionals, chronic disease management, emrs, use of non physician professionals: 1960s: fee for service model, clinical autonomy, physician control over the organization and place of medical practice. Innovations in organization and funding model including clsc in quebec and community health centers and health services organizations in ontario.
