Community of practice, may be working with community or discipline. When today"s community health nurses (chns) focus on the community as the unit of care, they recognize that the community has a unique identity with distinct health needs. An umbrella term includes community health nursing in a variety of practice areas, for example: Primary care nursing (specialized: provide first contact with healthcare, primary care occurs in primary care networks, ambulatory care centers, long- term care facilities, physician offices, family health clinics, community health centers, community clinical. Nurse manages and care for people with chronic diseases. Provide access to health information, counselling, health assessment, health promotion, and coordination of services. Supports principles of phc (accessibility, health promotion, use of appropriate technology) Focused care on building relationships while maintain dignity and respect for clients, creating an environment of cultural safety and empowerment.
