Think of gills as a major component. Each side of gill arch there are butterfly wing looking things. When water flows through them the filaments extend and accept the water flowing across them. In the diagram the green arrow is blood flow of poor in oxygen. The red arrow is blood flow of rich in oxygen. Water flowing through the mouth and gill structures, the water is coming through the arc and going across in the opposite direction of blood flow.
