The agency for healthcare research and quality (ahrq) define safety culture: The safety culture of an organization is the product of individual and group values, attitudes, perceptions, competencies, and patterns of behavior that determine the commitment to, and the style and proficiency of, an organization"s health and safety management. The patient is not only an active participant in his own care, but also acts as a liaison between the hospital and the community. Quality assurance (qa: quality assurance began as a method of inspection to ensure that health care institutions maintain minimum standards of care, qa measures performance against standards and reporting incidents and errors. This is a reactive approach and fixes the errors after a problem is noted. It emphasizes on doing it right and does little to sustain change or proactively identify problems before they occur. The focus of qa and qi are different.
