Document Summary

All likely auto-antibody related, multi-system with typical patterns of joint involvement, flares/ remissions. The ag is the synovium of the joints. Many theories why this develops, but result is certain tissues are considered as foreign, not self . Auto-ab develops for unknown reason- to synovium of joints but also other tissues. Auto- ab reacts to ag, forms an ag-ab complex, activates complement (c") C" directly damages synovium (pgs/ lt synthesized) and de granulates mast cells (h1 released) so pgs and h1: Wbcs digest ag/ auto-ab complex which is on the tissue surface so wbc damages tissue. Excess fibroblasts, collagen, etc. accumulates as part of repair, results in scar tissue build-up= pannus. Painful joints- nociceptors stimulated by chemicals and edema. Swollen (edema) boggy joints- inflammatory exudate in joints; increased vascular permeability. Joint stiffness- from swelling, pannus formation; pannus develops as scar tissue and immobilizes joint. Long term: deformity, nodules, erosion of joint, atrophy of surrounding muscle from non-use.
