Knowledge of the genetic regulate of viral resistance is dominated by variants that have a large effect hepatitis c infection places over 180 million people worldwide at risk of cirrhosis and liver carcinoma. These il28b polymorphisms are also associated with spontaneous clearance of the virus in chronically exposed individuals and with responsiveness to standard therapy. Only half of all people who receive pegylated interferon prophylaxis are able to clear the virus and individuals with the responsive il28b target genes (interferon-stimulated genes) in hepatocytes that mediate clearance . The variatns are now used clinically and may explain the differences in response rate between populations. Susceptibility to hepatitis b infection has to date been associated only with variation in mhc class ii, implicating viral antigen presentation to t helper cells. Cut slide: polymorphisms in the region of the gene il28b are immune response to hcv, not clear how the il28b haplotype affects hcv clearance, associated with hcv spontaneous clearance.
