Aim: to break down a blood clot, restore perfusion & reveal the vascular anatomy. Won"t be able to use an angiogram to visualize the vessels bc cm cannot get past the clot - cannot see downstream. Thrombolytic agent: delivered directly to the site - peripheral clots/infarcts directly to clot maximizes benefit to the patient. Acute limb ischemia < 3 days tpa (tissue plasminogen activator) - name when referring to thrombolysis. Ischemic right leg due to acute embolus in r femoral artery. Patient"s skin colour is bluish - sign that patient is exhibiting some kind of clot preventing blood flow to the area. Not uncommon for access to start to develop clots. Directly injecting a thrombolytic medication helps salvage the access point. If we have a bypass graft in the leg (ie), want to make sure graft is patent. Direct injection of thrombolytic agent helps to salvage that path. Peri-procedural thrombolysis - clot developing bc of a procedure.
