Overview information: 64 y/o hispanic female admitted 5/14 to emergency room with hypotension, dehydration and bp 58/42. Today (5/15): pt is on chronic hd 3 x per week, presents w/diabetic nephropathy. Medical hx: htn, hyperlipidemia, t2dm, cva w/right sided hemiplegia, anemia. Family hx: cva (brother), mi & dm (mother), renal disease, htn. Current medications/supplements: aspirin 81 mg, imdur 60mg, colace 100mg, folic acid, ditropan xl. 5mg, glucophage 250mg, glyburide 1. 25mg, lopressor 100mg, clonidine 0. 3mg. Medications prescribed/continued (5/15 hd#2): lopressor, clonidine, phoslo, epogen 10,000, Abw (using 97 kg & 61kg) 88kg (193. 6#) kdoqi. Pretibial edema in right leg (peripheral trace edema worse on right side) 24 hour recall: breakfast- cold cereal, bread or fried potatoes, fried egg, coffee lunch- bologna sandwich, potato chips, coke dinner- chili con carne, few tortillas, iced tea snack- crackers and peanut butter.
