Need to start coumadin several days before d/cing heparin to reach a therapeutic pt/inr level. 2. 5-3. 5 for patients at high risk for clots. Usually withhold or lower dose of coumadin if patient has high inr or bleeding. May need ffp (clotting factors) for quick reversal. Everything there is to know- complications, levels, interactions. Which lab value is associated with which anticoag.
