In p. a. potter, a. g. perry, j. c. ross-kerr m. j. Wood, b: astle, & w. duggleby (eds. ). Wound and pressure ulcer care - irrigating wounds and caring for pressure. In mosby (producer) mosby"s nursing video skills: intermediate skills. Pressure ulcer is localized injury to skin and underlying tissue, usually over a bony prominence, as a result of pressure, or in combination with shear and/or friction. Factors contribute to pressure ulcer development: pressure intensity, pressure duration, tissue tolerance. Capillary closing pressure is the minimal amount of pressure required to collapse a capillary (ex. when the pressure exceeds the normal capillary pressure range of 15 to 32 mm hg) Tissue ischemia can occur when pressure applied over capillary exceeds the normal capillary pressure and the vessel is occluded. Decreased sensation and thus does not have the cue of discomfort to prompt shifting (off- loading) pressure, tissue death may result.
