-Good morning! Hand Hygiene My name is Julie and I am a level one
nursing student at McMaster. Can you state your full name for me please?
And, how would you like to be addressed? Today I will be with you for about
40 minutes, doing ___ so before I begin I just want to remind you that
everything that we talk about today will remain confidential and only be
discussed with the rest of the health care team, providing your care. Are you
comfortable where you are? Okay, if at any point you become uncomfortable
let me know. As well, if you have any questions throughout, you can let me
know and ill be sure to answer. Can I please get your age and birthdate
please? And where do you live currently? Thank you! So what brings you in
today ___?

Notes:
Do not cross legs
Use name more than once
Be aware of non-verbal cues
Be loud and clear
Keep eye contact throughout
Use open-ended questions
Working Phase
-Completing tasks assigned, demonstrating active listening, asks questions,
clarifies, paraphrases patients words
Termination Phase
-Summarizes the key points discussed and asks patient if I have covered
everything accurately, offers actions of follow up, asks the patient if they
have any questions or concerns, smooth transition Hand Hygiene

Orientation Phase
For the moment I stepped into the room, I started the general survey.
Assess Physical Appearance:
-Age, appears the stated age
-Sex, development is appropriate for age and gender
-Signs of Distress
-Level of Consciousness, alert, oriented, attends to questions, responds
appropriately
-Skin Colour, even tone, pigment consistent with genetic background, intact,
no lesions
-Facial Features, symmetric with movement and at rest
Overall no signs of acute distress: pain, grimacing, guarding, short of
breath, anxiety.
Assess Body Structure:
-Stature/Body Type, height in normal range for age & genetics
-Nutrition, weight in normal range for height, even fat distribution
-Symmetry, body parts equal bilaterally and in proportion
-Posture, standing comfortably (line through anterior ear, shoulder, hip,
patella, ankle)
-Position, sitting comfortably, arms relaxed, facing examiner
-Body build/contour, arm span=height Can you please stand up for me
and spread out our arms?
-If there is any obvious physical deformities, no congenital or acquired
defects.
Assess Mobility:
-Gait, Can you please get up and walk to the other side of the room
near ___, and then walk back?, while they were walking I looked if their
feet were shoulder width apart, feet placed accurately, smooth and balanced
walk, symmetrical arm swing
-Range of motion, From the moment they moved from the chair I was
observing for full mobility in joints, deliberate, smooth and coordinated
movement
