MENTAL STATE EXAMINATION (MSE) TEMPLATE

1. Appearance – Well groomed, height, skin colour, hair colour, eye colour,

hygiene, age

2. Behaviour – restless and action, agitation, anxiety, mannerisms, eye contact,

stride/gait, fidgeting, posture, withdrawn, suspicious behaviour (paranoia)

3. Speech – coherent, rate, pitch, tone

4. Mood – ask how they’re feeling, (needs to match affect), patients opinion on

mood not necessarily their appeared mood

5. Affect – the way they look (mood), is it congruent with stated mood?

6. Thought Form – disconnected sentences, ideas, words, relevance, sentence

flow, neoglisms (Made up words)

7. Thought Content – suicidal, delusional, paranoia,

8. Perceptual – hallucinations

9. Cognition – orientation (time & place), MMSE, GCS

10. Insight (& judgement) – do they know why they are there? do they know about

their illness?
