Form CMS-1500 is the standard paper claim form used to bill an insurance company for rendered services and supplies. It provides detailed information about the patient, his/her insurance policy(s), and his/her diagnosis and treatment. This assignment will require you to analyze the content, the format, and the history of the form.

Instructions

You will write a 4–6 page research paper that provides a contrast between the CMS-1500 (08/05) and the CMS-1500 (02/12).

For this assignment, include the following:

 

• The history of the CMS-1500 form.

• A description of the changes to the format.

• An analysis of why the changes were made.

 

You may use materials from the course, scholarly sources, and Internet sources; however, you must be sure that the source material is providing correct information. You must cite at least 4 references, and you must use current APA format.

Patient Rights in the American Healthcare System.  Visit a hospital website – you may choose a hospital you are familiar with, that is in your community or a nationally recognized health system like Mayo or Geisinger.  On the hospital website, search for patient and visitor information about Patient Rights and Responsibilities. 

In your discussion post, answer the following questions:  Was the information easy to find?  Was it easy to understand? Was the information provided by the hospital consistent with the rights described in the Torrey article assigned?  Do you think that these lists are adequate or complete? Is there a right or responsibility you believe is missing from the lists you have read? 

To better the rural community it serves, XYZ Hospital has instituted a remote patient monitoring system for patients with pacemakers. Patients use a smart phone app to download the information collected by the pacemaker that is then sent to their physicians on a more frequent basis than a regularly scheduled doctor's appointment.
