Summarize what you talked about with the patient and ask for anything that could"ve been missed. Remember the assessment is ongoing - each time you see the patient is an assessment! Assess how the patient is perceiving their current condition. Assess substance use and any history of substance use disorder. Assess the client"s stress level and ability to cope with stress. Assess the clients cultural beliefs regarding health care and their values. Stuporous: the need to use painful stimuli to waken the client. Decerabrate: neck/elbow extension, wrist and finger flexion. Mood: the emotion the patient is feeling. Affect: an objective (something we can see) expression of the patients mood - a frown. Orientation questions - person, place, time, situation. Immediate: list a series of objects and have the patient repeat.
