John: jim has a healthy heart with very little plaque build up; john has significant blockage in several arteries. Blood pressure: bp measured in the brachial artery, sbp reflects work of the heart, tpr the resistance to flow from the heart is reflected in dbp, someone with hypertension usually has an elevated dbp. What does this mean: the resistance to flow from the heart is high and this makes the heart work harder to pump blood to open the aortic valve, therefore, sbp has to rise too. Cad - the role of hypertension: high blood pressure promotes atherogenesis by two methods, it can damage the inner lining of arteries and cause cholesterol and wbc"s to be attracted to the area, damage = inflammation. This causes the formation of oxygen radicals: so, hypertension is a major risk factor for cad and when combined with other risk factors (chronic inflammation, high ldl), atherogenesis can be accelerated.
