Pharmacotherapy work up is a systematic approach to identify problems related to patient"s drug therapy and make rationale drug related decision. That all of patient"s medications are indicated, effective, safe, and that the patient can be adherent to their medication. Dtp occurs when drug related needs are not met. Indication: decisions concerning indication must be addressed first that is unnecessary drug therapy or needs additional drug therapy, effectiveness, then decisions concerning effectiveness must be established. Example: the patient is taking2 nsaids, ibuprofen and naproxen for pain control putting the patient at risk for drug toxicity: needs additional drug therapy to treat or prevent a medical condition. Common causes are: medical condition requires the initiation of drug therapy, preventative drug therapy is required to reduce the risk of developing a new condition. Example: the patient is at high risk to contract pneumonia and therefore requires a pneumococcal vaccine: the drug is not effective at producing the desired response in the patient.
