Thyroid store: acute, life-threatening, thyroid hormone-induced hypermetabolic sate. Symptoms include fever, tachycardia, neurological abnormalities, hypertensions followed by hypotension & shock. Thyroid storm id left untreated is invariably fatal, thus rapid diagnosis & aggressive treatment is necessary. Infection, surgery, trauma, radioactive iodine treatment, pregnancy, anticholinergic. & adrenergic drugs , th ingestion, diabetic ketoacidosis (dka) Manage w/beta blockers, calcium channel blockers deals with high blood pressure. Digitalis or diuretic decrease blood pressure by increase volume of liquid removed from body. Potassium iodide (pima, sski, thyroid-block) used to inhibit th release from thyroid gland. Propylthiouracil (pt) inhibits synthesis of th by preventing organification and trapping of iodide to iodine/inhibition of coupling idootyrosines. Also inhibits peripheral conversion of t4 to t3. Methimazole inhibits synthesis of th by preventing organification of iodide to iodine & coupling idotyrosines. Decreases hr, myocardial contractility , blood pressure and myocardial oxygen demand. Its often necessary drug to control symptoms of thyroid storm.
