So actually a disorder of conjugating capacity since 3-5% of the population are affected by this, gilbert"s syndrome clearly doesn"t lead to progressive disruption of hepatocyte and progressive liver disorder. But it can fool you b/c you end up with 3x increase in bilirubin levels. Bilirubin levels rise within the hepatocytes until they can push through the modest enzymatic block due to high capacity. Then, its handled by gut & even if the bilirubin does return, the liver can handle it well. So, some bilirubin backs up but the conjugated bilirubin gets through and doesn"t spill over into the urine. Urobilinogen is adequately handled b/c not that much bilirubin gets through into enterohepatic circulation & at this point, the hepatocytes aren"t that sick: are the causes of unconjugated hyperbilirubinemia benign? system. Suggested that hemolytic anemia will give you a high bu & this is not particularly benign. Also suggested that gilbert"s syndrome can do the same.
