Planning phase: prioritize issue raised during assessment (nursing diagnoses) Intervention phase: plan of care is carried out. Evaluation phase: the plans outcome would be assessed to see if outcomes were reached or nursing intervention need revision. Nursing theory would say something about every concept. Environment: internal and external aspects (anywhere therapeutic relationship occurs, involve family, social ties, community, health care system, geopolitical issues, how to work in larger context of every experience of wellness and illness. Health: focus of our care, not just in clinical manner, unity of mindbodyspirit, improve health of individual and society, how you precieve and experience yourself, reach outcomes that were productive and satisfying despite disease, wholeness. Nursing: compassions, ethics, how we chose to be with person, understand client with environment, life and health goals, promote harmony in mindbodyspirit, transpersonal. Caring is more important to the client than nurse.
