Quantitative evaluation of a pediatric rheumatology transition program. Jensen, p. t. , karnes, j. , jones, k. , lehman, a. , rennebohm, r. , higgins, g. c. , spencer, c. h. , Pediatric rheumatology, 13(1). doi: 10. 1186/s12969-015-0013-0 (jensen et al. , 2015) Some challenges that occur after transition include difficulty in navigating the adult healthcare system, lack of self-efficacy, and limited experience regarding self-management of their illness. Interruptions in care can lead to a negative disease impact. There have been guidelines developed for the formal transition process - including being individualized and multidisciplinary. In rheumatological practices, the process of transition is difficult. Communication needs to be strengthened during and after the transition process. There are very few studies that analyze the results of transition programs and are quantitative. Many specific transition models have been described. Transition success is seen as patients who saw an adult rheumatologist at least once. When compared to patients who participated in the program, control patients did not have as successful transitions.
