Def: the conscientious, explicit and judicious use of the current best evidence in making decisions about the care of individual patients. Improved patient outcomes: patient"s values and expectations, individual clinical expertise, best available clinical evidence. It"s the way it"s always been done why change it: authority, textbooks become outdated quickly and its important to replace them, clinical expertise; trial and error; intuition.
