Stratum corneum is thin outer layer with latened kerainized cells. Provides tensile strength, support, and protecion to bones, muscles & organs. Localized injury to skin/underlying issue caused by pressure, shear/fricion. If pressure applied over capillary exceeds the normal capillary pressure for a prolonger period, issue ischemia occurs: if sensaion is reduced, paient may not feel this/ get the cue to change posiions. Ater ischemia period once blood low returns, area is reddened (blanching hyperemia, will blanch when pressed) If area does not blanch when pressed, may have deep issue damage (non blanching erythema) Other factors like age, nutriion, low blood pressure also decrease issue tolerance. Uses 6 scales: sensory percepion, moisture, acivity, mobility, nutriion and fricion/shear. Include percentage of non viable issue, and appearance. Granulaion issue is red, moist, indicates progression of healing. Slough is sot yellow/white and stringy (must remove for wound to heal) Eschar is black/brown necroic issue (must be removed to heal)
