Included under extremities ext exam in section of soap note but not limited. Normal finding: (-) cce: no cyanosis, clubbing, or edema. Inadequate oxygenation of blood (i. e. hypoventilation from opioid overdose) Increased bilirubin due to hepatic dysfunction (cirrhosis) Decreased turgor tenting decreases turgor in dehydration (i. e. edema (+) edema diuretic use, vomiting/diarrhea) Small non-raised patches due to bleeding under the skin. Causes: blood outside the vessels/bruising (i. e. warfarin), trauma, bleeding disorders. Irregular, transient, superficial areas of localized edema. Causes: hives/urticaria from allergic reactions (i. e. food, insect bites, drugs) Circumscribed, pus-filled, elevated lesions of varying size. Dilated small surface arteries that appear as small red spots with multiple radiating arms. Increased angle (>180) between base and bed, thickening of finger tip. Proximal of nail plate appears white, whereas distal appears red. Inspect chest at least one complete inhale-exhale cycle.
