There is a 5-step nursing process and a 6 step nursing process. Assessment stage- gather the information about the patients conditions. Compare the clinical findings with normal and abnormal variations and developmental events. The problem is validated by the presences of major defining characteristics. Consist of the diagnostic label, contributing factors, and signs and symptoms. Ex; constipation related to inadequate fiber and fluid intake as evidenced by reports of dry hard stool every 3-4 days. Not taking in enough fluid and fiber can make you irregular in bowel movements: possible nursing diagnosis- statements that describe a suspected problem requiring additional data. Consists of 2 parts- the possible nursing diagnostic label, and the contributing factors that lead the nurse to suspect the diagnosis.
