Assess if patient in need of prn analgesic/opiod. 1710 1730 medications due at 1700 and 1800. 1730 1800 ask patient if would like to ambulate. Try to get patient into chair for dinner if they agree to. Reassess pain to see if analgesic is working. Flush with 10ml of sodium chloride then lock with 3ml 4% sodium citrate. 1930 2000 change dressing if soiled and attend to any comfort measures for patient. 2000 2030 medications due at 2200, but might give at this time because patient falls asleep around this time. Ask if patient would like prn pain medication before sleeping, if due for another dose. 2030 2045 check chart for any new updates throughout shift so far.
