A 26-year-old man has been brought to the emergency department after a motor vehicle collision. His injuries include a fractured right femur and blunt force trauma to his abdomen with an open laceration. He is bleeding from the abdominal wound and it was reported that he lost a lot of blood at the scene. A large bore iv was started by the ambulance crew and he is receiving iv 0. 9% normal saline. A pressure dressing has been applied to his abdominal wound, but blood is already showing through, saturating the dressing. His vital signs are: temp 36 oc, hr. 102, resp rate 22, bp 98/78 mmhg, o2 sats 92% on room air. He is awake, alert, and anxious and it is reported that there was no loss of consciousness. He is yelling out in pain and describes the pain being most severe to his right leg with a pain rating of 20 out of 10 .
