Erect chest x-ray is preferred to a supine cxr . Diaphragm is at its lowest position (gravity) Other anatomy is away from the area of interest. Back of hands low on hips positions the scapulae laterally so they are clear of the lung fields. Exposure is made on arrested full inspiration. Important that the patient is observed during exposure to ensure that the image is taken during full inspiration. Expiration on the left, inspiration on the right. Need to be out of the way of the lung fields other wise they may mimic pathology. Alternative arm positioning if patients are unable to put the back of their hands low on their hips, they can wrap their arms around the bucky stand infront. Get patient to internally rotate the arms so that the thumbs are pointing downwards to help roll the scapulae of the image. Patient must be against the ir and not leaning forwards this will lead to chest foreshortening.
