Predisposing factors: family hx, frequent episodes of laryngitis, prolonged alcohol use, tobacco use, exposure to chemicals, poor diet, risk increases with age. Sx: oral ulcer that does not heal, hoarseness, foul breath, unilateral ear or throat pain, hard immobile neck lymph nodes, weight loss, airway obstruction, lump/mass in neck. Dx: h&p, no routine test, so usually not known until late stages, ct, mri, pet, laryngoscopy: scope passed through mouth into larynx in order to visualize mass. Fast 6 hr prior: needle biopsy: gold standard. Concentrated radioactive source into or near tumor: chemo. To get rid of cancer cells: surgery. Total: will have permanent tracheostomy bc no connection between upper and lower airway. Total laryngectomy and radical excision of lymph nodes and tissue: nutrition. Pt is already malnourished and treatment increases malnutrition risk. Refrain from smoking and excessive use of alcohol. Leave me alone; i"m so tired of talking i can"t see straight.
