Assessment of skin, hair, and nails: structure of the skin. Made up of stratum germinativum or basal cell layer and stratum corneum or horny cell layer. Nail- full nail is regrown every 3 months. G. wound repair- surface wounds repair in 7-10 days, deeper wounds take ~4 weeks to heal. Previous history of skin disease- psoriasis, hives, skin cancers, sensitivity. Lecture 4: objective data the physical examination. C. inspect and palpate nails: shape and contour- is nail smooth, rigged. Profile sign (where the cuticle meets the nail, it is flat. Common in congenital heart disease and copd): consistency- thick vs thin, color- cap refill (push down on the nail and it should turn red again within 2 seconds) Typically cancerous moles are asymmetrical, no defined border, darker, elevated, E. abnormal findings: common shapes and configuration of lesions. F. abnormal findings: primary and secondary skin lesions. Primary skin lesions a. macule- flat, less than 1 cm diameter, erythematous.
