What else can we do to identify causality?
Not feasible to do RCT examining the effect of exercise on disease endpoints (CHD,
stoke, death)
However – we can look to see if exercise influences known CVD risk factors
Lee & Blair (2000)
◦Compared with the least fit quintile (#1)
◦Quintiles 2 and 3 had 0.37 times the relative risk of stroke mortality
◦Quintiles 4 and 5 (most fit) had 0.32 times the relative risk of stroke mortality
◦However only 32 cases of stroke mortality
were documented. N’s are very small:
14 (low fitness) vs. 11 (moderate fitness) vs. 7 (high fitness group).
Blood pressure: results from RCTs
Meta-analysis of 54 RCTs (only difference on groups was aerobic exercise)
Average reduction in systolic BP: 3.84 mm Hg
Average reduction in diastolic BP: 2.58 mm Hg
Reductions were noted in hypertensive and normotensive; and in overweight and normal
weight participants*
Note that this is what was found in MOST studies – not all studies found the same thing
