Problem based history: movement, how long, are joints swollen/hot/stiff, muscle weakness, knees/ankles give way, adl. Shoulders at same level: muscle spasms with movement, rom of thoracic and lumbar spine, turn/twist side to side, touch their toes, hands on hips and bend to test lateral flexion. Inspect shoulder girdle: symmetry of location/shape, bony prominences, palpate shoulders for tenderness/masses, palpate elbows for tenderness, swelling and rom, dominant side may be larger , check strength/rom, straight extension, back extension, abduction, adduction, shoulder rotation. Inspect knees for symmetry & do they align: line up with tibia, do ankles have lateral of medial deviation. Joint itself should be smooth & non tender: note any edema/warmth/rom.
