External genitalia:
Vulva which encompasses:
mons pubis
labia majora
labia minora
clitoris
vestibular glands
vaginal vestibule
vaginal orifice
Urethra
1. Mons pubis: a pad of adipose tissue covering the symphysis
pubis. Terminal hair covers the mons pubis and extends down the
labia majora in the post pubescence female. (more adipose tissue
and less terminal hair as we age)
2. Labia minora lies inside the labia majora, and is hairless, flat, and
reddish. Anteriorly the labia minora meet at the anterior vulva and
divide and form the frenulum of the clitoris, and the prepuce of the
clitoris. The clitoris sits between the frenulum and prepuce and is
the counterpart of the male penis.
3. There are 6 openings in the vestibule. Two skene's glands on
either side of the urethra, 2 Bartholin’s glands posteriorly on either
side of the vagina.
4. The urethra lies between the clitoris and vagina; the vagina lies in
the posterior vestibule. The vagina is varied in size and shape.
Hymenal connective tissue covers some portion of the vagina, and
when torn leaves hymenal tags.

1. Vagina: musculomembranous tube that is transversely rugated
(forms ridges on the wall) during reproductive years. Lies at an
angle of 45 degrees with the vertical plane of the body. The upper
end of the vagina is a blind vault which ends in the cervix, which
protrudes into the vagina, and pockets of the posterior, ant, and
lateral fornices. The walls are thin and make it possible to examine
organs through these walls.
2. Uterus: sits between the rectum and bladder. Inverted, pear
shaped and relatively mobile. Covered by the peritoneum, and
lined by endometrium which is shed during menstruation.
3. Retro uterine cul-de-sac (pouch of Douglas): is a deep recess
formed by the peritoneum which covers the lower posterior wall of
the uterus and upper portion of the vagina.
4. Uterus usually inclines forward at a 45 degree angle although may
be anteverted, anteflexed, retroverted, or retroflexed.
5. Size of the uterus of a nulliparous woman is 5.5-8 cms long, 3.5-4
cms wide and 2-2.5 cm thick. The uterus of a parous woman may
be 2-3 cms larger in any dimension. (DON’T NEED TO KNOW
FOR EXAM)
6. Uterus divided anatomically into the corpus and cervix. The corpus
is divided into the fundus, body and isthmus. The fallopian tubes
are inserted at the fundus of the corpus.
7. Adnexa of the uterus is comprised of the fallopian tubes and the
ovaries. Each tube ranges from 8-14 cms. The ovaries rest in a
depression of lateral pelvic wall at the anterosuperior iliac crest.

During the reproductive years the ovaries are about 3cm long, 2
cms wide, and 1 cm thick.

History of Present Illness
Abnormal bleeding: less than 19-21 days or more than
37 days between periods.
Nature of flow, temporal sequence, associated
symptoms, pain, efforts to treat
Vaginal discharge: character, occurrence, docking,
clothing, discharge in partner, use of condoms,
associated symptoms.
Endometriosis
PMS: This is real, what are the symptoms (pain,
fatigue, diaphoresis, cravings, etc), what have they
done.
Menopausal symptoms: age at menopause (around
51-52), symptoms, postmenopausal bleeding, self-
image (don’t feel like they are a woman anymore, can’t
bear children), Mothers experience, meds, include
contraception if currently experiencing.
Infertility: how long trying to get pregnant, abnormal
structures, diagnostic Evals to date
Urinary symptoms: character, description of urine,
associated symptoms, related to sexual activity,
method of contraception.
Women are more likely to get UTIs because they have
shorter urethras and they hold their urine
