Under normal circumstances, pde-5 breaks down cgmp back down to gmp, terminating the effect, but the ed drugs take out pde-5 so we"re not breaking it down into gmp, and we have too much vasodilation. Concurrent administration of the organic nitrates and ed drugs causes an increase in intracellular cgmp and an exaggerated decrease in blood pressure. If you have angina and need drugs for your ed, wait 6 hours between the time you took the nitrates and the time you will need the erectile dysfunction drugs. That way, men are able to use both drugs without the risk of hypotension or myocardial infarction.
