Systolic bp greater than 180 mm hg and/or diastolic. Difference between urgency and emergency is absence or presence of target organ damage. Urgency: develops over hours to days and does not have clinical evidence of target organ disease, may not require hospitalization. Emergency: target organ disease and most often require hospitalization. Htn crisis r/t cocaine or crack use is frequently seen. Necessary in severe cases e. g. gangrene, osteomyelitis. Pre-op teaching: exercises to strength upper extremities, phantom limb sensation. Post-op care: below the knee/above the knee amputation. Positioning to prevent flexion contractures: avoid sitting in chair for more than 1 hour at a time, lay on stomach 3-4 times a day, no pillows under surgical extremity. Inspect residual limb daily for signs or skin irritation: wash limb with warm water and bacteriostatic soap daily and let dry for 20 mins, wear clean limb sock, positioning to prevent contractures. Exchange of substances between tissues and blood.
