The middle carpal joint is between the proximal and distal row of carpal bones (blue line). The carpometacarpal joint is between the distal row of carpal bones and the metacarpal bones (green line). The flexor surface of these three joints is their palmar aspect (curved arrow in lateral schematic). The antebrachiocarpal and middle carpal joints are the most mobile in the carpus, there is very little movement at carpometacarpal joint. Tap the dorsal view of the carpus below to toggle the carpal joint capsule and understand the following: the carpal joint capsule extends like a sheath across the three articular levels. The antebrachiocarpal compartment does not communicate with the compartment of the two distal joints. The middle carpal and carpometacarpal compartments communicate with each other, but not with the antebrachiocarpal compartment. The carpal joint is located topographically by palpating the accessory carpal bone.
