Could be converted from existing multispecialty medical groups, physician- hospital organizations (pho), and organized or integrated delivery system, or tightly managed independent practice associations (ipas). Providers" incomes are linked directly to the volume and intensity of services, rewarding growth and penalizing efficient providers who reduce costs. Decouple provider incomes from volume and intensity; pay for better value: improved health and outcomes, better quality, reduced costs.
