Set of health care providers that work together and accept collective accountability for the cost and quality of care delivered to a population of patients. Could be converted from existing multispecialty medical groups, physician- hospital organizations (pho), and organized or integrated delivery system, or tightly managed independent practice associations (ipas). Blues, cigna, anthem, aetna all supporting their own versions. Fostering accountable health care: moving forward in medicare fisher, et al. Fragmented payment and delivery system provides little support for coherent local decision making on local capacity and lacks accountability for quality and efficiency. Foster local organizational accountability for the continuum of patients" care outcomes, quality, and costs and for the efficient management of current and future health system capacity. Providers" incomes are linked directly to the volume and intensity of services, rewarding growth and penalizing efficient providers who reduce costs. Decouple provider incomes from volume and intensity; pay for better value: improved health and outcomes, better quality, reduced costs.
