Global Health Systems
• Why did it take ebola to realize importance of health systems
• Most public health have a disease/interventions focus rather than a systems focus (theyre
messy)
• Paul farmer:
• Health systems and econ consequences of ebola epidemic
• Liberia
o Ppl were afraid to come to the hospital/health officials
o 65% + facilities were closed
• Were only 20+ liberian physicians so this was a big deal
o More than 600 healthcare staff in liberia and sierra leone have been infected and most
have died
o Lost 2.1% points in GDP in guinea, 3.4 in liberia, 3.3 in sierra leone
• Resilient health systems
• That are able to absorb the shock of an emergency like ebola
• Also continue ot provide regular health services
• Approach of global community in wake of ebola: multi sectoral, integrated approach that
equips countries that equips countries to absorb unforeseen severe shocks
• Puerto rico now: arent getting meds, have no power for surgery, etc.
• Money flows where the interest lies
• Challenge of global health
• Biggest problem seemed to be lack of resources, but now there is a rise in public/private
giving
• Historical context:
• Organized health systems barely existed a century ago
• Industrial revollution: workers health became a political issue (capital)
o Bismark germany 1883: state mandated social insurance model
o Spread to other european countries, japan
• WwII: beverdidge report of 1942: national health service 1948
o Many former colonies adopted beveridge model
• Most health systems today are one of these two models from 19th century
• Bismark or beveridge ( vs health services in the public sector)
• Seminal report: world health report, health systems: improving perfomance
• What is a health system?
• Those activities whos primary purpose is to promote, restore, or maintain health
• If a government were to invest in healthcare systems, what should it invest in?
• Accessibility (transportation, availability of physicians,)
• Health work force (helps w accessibility issue)
