A mood disorder involves pathologic disturbances of the internal emotional state. Affect is the external manifestation of the internal emotional state. Negative attitudes, e. g. belief that it is impossible to improve their lives. Anhedonia (derives no pleasure from normal things of life) Belief a body part is diseased or rotting; chronic pains. Psychomotor retardation (slow movements & response to questions) Believed to involve abnormal biogenic amine levels in the brain. Amines released in brain, taken up by same cell via transporter. Packaged into vesicles or mao in mitochondria degrades. Blocks reuptake of norepinephrine and 5ht into nerve terminals. Toxic: potentially fatal in combo with mao inhibitors. Toxic: combo of mao inhibitor plus tri- or tetracyclics (high fever, delirium, hypertension) Hypertensive crisis: combo with red wines, cheese, pickled fish (tyramine) Affects 9% of population at some time in their lives. Outcome goals: prevent suicide, reduce duration of depression and frequency of recurrence.
