Acute infection = tonsils bright red, swollen and may have exudate with large white spots. White membrane covering the tonsils = infectious mononucleosis, leukemia, diphtheria. Press on posterior wall to elicit gag reflex test for nerves ix, x (glossopharyngeal and vagus nerve) Surface peppered with indentations or crypts (may collect small plugs of white cellular debris) Push on tip of tongue should hunch back. Slightly press down on center but avoid eliciting gag reflex. Scan posterior wall for color, exudate or lesions. Stick out tongue (note tremors, loss of movement or deviation to side) Damage xii = tongue deviates to paralyzed side. Diabetic ketoacidosis = sweet breath odour acetone breath. Acetone breath also occurs with small children with malnutrition/dehydration. Oral examination usually done at the end, because intrusive to infants and children. Ask parent to position child for you. Playing games with child can increase compliance.
