Micro-Allocation
•
A justifiable scheme for allocating medical resources must involve two
components
1)
Realistic assessment of whether this resource will help this specific
patient in a way that means its being used wisely and efficiently
2)
Respect patient rights while also respecting the needs and interests
of the institution and greater patient population
•
First come first served is fair, but not efficient
•
Glannon argues we are responsible for the harm we cause ourselves (ex:
liver failure from drinking) should not be treated equally
Allocation of Funding
•
Distinction between basic needs and wants
•
Healthcare is a basic need, therefore, it should always be preferred over
museums, hockey rinks, etc
•
Counter arguments
1)
Not all health care addresses basic needs, ex cosmetic surgery
2)
Spending only on healthcare until perfect health is achieved is not
attainable
•
Conflicts between kinds of care ex: emergency vs. neonatal care
•
Conflicts between present benefits and future ones
Public vs Private
1.
Romanow report
○
Argued in favour od maintaining the public system
○Largely public system is more sustainable
○Canada is a mix, 66.7% of total money spent on prescription drugs
come from private sources
2. Canada health act
○Sets criteria that each provincial insurance plan must comply with in
order to receive funding (federal): comprehensiveness, accessibility,
universality, portability, and public administration
○Focus on principle of justice
3. Chaoulli V Quebec
○In 2005, this ban on private health insurance for services that are
covered under the province's public insurance plan was challenged
○Government introduced wait time guarantees, pledging that when
patients waited longer than 9 months, they would get private care
at public expense
