Diseases which damage the glomerulus are among the most important cause of chronic kidney failure in humans. After studying this case you should: understand the role of immune reactions in causing glomerular disease (this is an example of a primary glomerulopathy - post-streptococcal glomerulonephritis). This is also a good opportunity to review your knowledge of immune disorders and hypersensitivity reactions, particularly type ii and type iii. This example is typical of type iii hypersensitivity, i. e. the deposition of circulating antigen-antibody complexes within the kidney glomerulus and activation of the complement pathway. A nine year old boy had a flu-like illness and sore throat. He was treated with antibiotics for 10 days. Two weeks later, after a hockey game, he developed nausea, malaise and a high temperature (39 c). He went home later and noticed that his urine was red. His parents took him to their family doctor.
