If lifestyle changes are needed then you need support of all family (ex. Low sodium diet, need wife to stop cooking with salt: embrace what each pt sees as family. Discuss: with the patient and family discuss five key areas to prevent problems at home. 1. what life at home will be like: depends of what you have.
